DEARhealth is Bending the Curve of the Growing Crisis of Chronic Disease.
The DEARhealth software is based on clinical medicine, where healthcare providers and their patients with a chronic condition are supported in the continuous care management. The treatment for these diseases is structured in care pathways: a timeline of a certain duration e.g. 12 months, where patients and healthcare providers can see an overview of planned care activities to manage their disease. Clinical decision support is provided by DEARhealth to the healthcare provider in determining the best fitting care pathway for an individual patient through its Recommender functionality.
Care pathways are initially based on disease-specific scenarios (e.g. low vs high risk) where a certain pattern of care activities (e.g. clinic visits, lab tests, procedures) is pre-modelled based on clinical (inter)national guidelines and medical/nursing expert input. These pre-modelled scenarios form a baseline care pathway for an individual patient, which can be further modified (removing or adding activities) by their healthcare provider as they see fit. Several data (e.g. patient or provider questionnaires, clinical data integrated from electronic health/medical record (EHR/EMR) systems, etc) are used as input for the Recommender to 1) point towards the best fitting care pathway scenario and modifications, 2) adjust the care pathway to new developments in a patient's health status due to continuous monitoring. The healthcare provider can access this information and resulting recommendations for treatment purposes. They must approve or deny a recommendation (e.g. change a patient's care pathway from low risk to high risk scenario) and are also alerted about certain outcomes (e.g. worsening of the patient's health score) so that they may be aware and make relevant adjustments to that patient's care pathway (e.g. plan an extra consultation).

After your healthcare provider has prescribed DEARhealth for you, you will receive an e-mail with instructions. Next step is to download the DEARhealth app from the Play Store of Google or from the App Store of Apple. Install the app and open it.
In this video is a step-by-step guide of how to log into the DEARhealth app. This procedure is a one time only task. After finishing these steps, you can login via a PIN code or biometrics of the smartphone.
Please note that because of security reasons, after 30 days not logging into the DEARhealth app, again these steps are necessary to follow.
Step 1: Open the app. Tap on the "Login" button in the welcome screen.
Step 2: Enter the email address which you provided to your healthcare provide, then tap on the “send code” button.
Step 3: You will receive an email with a verification code. If you are opening the email on your mobile phone, you can tap on the “log in” button to be redirected to the app again. If you are opening the email via an external device (such as your computer), you will need to open the app on your smartphone and type the code manually. Please note that the verification code will expire in 3 minutes. When this time is passed before you use the button to login or manually entered the verification code, you start from the beginning at step 1.
Step 4: You will receive another verification code via SMS on your smartphone. Enter this code and click on 'Confirm'.
Step 5: Next, you need to create a PIN code for your account. Create a code that is strong but easy to remember.
Step 6: The next step is to agree to the terms of use. Click on 'Confirm'.
And it’s done! On your next login, simply enter your pincode or biometrics (Face ID or fingerprint) to open the app.
That's okay, it happens! To reset your pincode, tap on "Forgot PINcode"? in the app and follow the steps. You have to repeat the steps, like you did the first time you opened the app.
Log in using the email address which you provided to your healthcare provider. Make sure there is no typo in your email address.
Still not successful? Contact your healthcare provider and ask them to verify the email address you are using is registered for DEARhealth.
The verification code is only valid for 3 minutes and can only be used once. Each time you log in, you will receive a new verification code. Check the date and time you received the email. The email should not be older than 3 minutes.
Still not working? Close the app, open the app again, and enter your email address again.
Check that you entered your email address correctly in the app. Close the app, open it again and enter your email address again. While entering your email address, pay close attention to characters such as _ and - and dots.
If, after a few minutes, you still haven't received a code then it may be that your healthcare provider has noted your email address incorrectly. Please contact your provider to check your registered email address.
DEARhealth uses the notification feature of Apple and Android. We advise you to activate the push notifications for DEARhealth to not miss any update from your provider and get reminder to provide input about your health status.

DEARhealth is processing personal data on behalf of your healthcare provider. This means that your provider is responsible for which data is being processed and for which exact cause. Of course your permission is always needed for processing data about your health.
At this moment only providers are able to send messages to you, you cannot reply. This functionality will be released later this year. You can contact your provider via the standard protocol of your provider.
Click on the button with the three lines at the top left and navigate to the main menu. Here you will find contact details and you can also log out of the app.
In the main menu you can navigate to settings, where you can view your personal details as known to DEARhealth and change the language of the app. Currently the app is available in Dutch and English. Note: Not all content in My Well-being and My Academy may be available in English, this depends on available information from the hospital.
If you try to save your questionnaire and it is not working, try to hard close the DEARhealth app and try to save it again. If that is not working, try to cancel the questionnaire and start a new one. Still not working, please contact DEARhealth at support at dearhealth.com and describe your problem. We try to fix it as soon as possible.

DEARhealth has an agreement with the hospital.
This agreement stipulates that if you want to quit using the DEARhealth patient app, you can inform your healthcare provider in the hospital.
Your data as processed on the DEARhealth platform will be transferred to the hospital, and removed from the DEARhealth platform. Data is also being removed, upon termination of the agreement with the hospital.
You can uninstall the DEARhealth app from your phone.

The DEARhealth portal is accessible via the EMR (Note: Not every provider has chosen to implement this functionality. Pay attention that the browser of the EMR vendor, complies to the supporting browsers). Next to the entrance via the EMR, the DEARhealth platform is also accessible stand-alone, see also the Login option in the menu on this website.
Stap 1: Open the URL for the DEARhealth portal (also accessible at the login option on this website). Choose your Healthcare provider.
Step 2: Enter the credentials that you received by e-mail.
Step 3: When logging in for the first time, you must set your own password, choose a password with at least 8 characters, of which at least: 1 uppercase letter, 1 lowercase letter, 1 number and 1 special character and click on 'Renew password'.
Step 4: You will receive a verification code via SMS, enter this code and click on 'Confirm'.
Log in using the email address you provided to DEARhealth. Make sure there is not a typo in your email address and that you have chosen the right healthcare provider in the first step.
Still not successful? Then contact us at support at dearhealth.com or ask your superuser from your organisation.
Is it your first time login? Please use the temporary password from the email you received. If this email was sent more than 30 days ago, your temporary password is not valid anymore. You have to contact your super user or DEARhealth to reset your password to receive a new temporary password. (Forget password will also not work).
Is it your second or more time login? Check that you have written your password correctly. Think about the conditions, 1 Capital letter, 1 small letter, 1 number and 1 special symbol. Still not working? Choose the option forgot password to create a new password. You will receive a code via e-mail before you can login with the new password.

If a patient experiences problems using the app, they can report this to the DEARhealth support desk or to their own healthcare provider. In some situations it may be necessary for the DEARhealth support team to gain access to your production environment. In such situations, this is done based on the internal patientID and not based on the actual name and address details of the patient. The internal patientID can be looked up by a healthcare provider in the URL of the healthcare provider portal and this ID can then be passed on to DEARhealth.
